Task

John is a 27 year old office worker that plays football recreationally on a Sunday morning. During a recent match John experienced a sudden feeling of dizziness along with the sensation of a very rapid heart beat in his chest. The sensation was transient and soon resolved however John was concerned and booked an appointment with his local General Practitioner (GP), who subsequently referred John to a Cardiologist for investigation.
John underwent a 12 lead ECG, a 24 hour ECG ambulatory monitor and an echocardiogram at the hospital before meeting with the Cardiologist. The 12 lead ECG revealed an unusual pattern of hypertrophy, and the echocardiogram showed evidence of asymmetric left ventricular hypertrophy specifically in the area of the basal interventricular septum. When asked by the Cardiologist, John explained that his uncle and grandfather had both died suddenly without warning. The Cardiologist subsequently diagnosed John with Hypertrophic Cardiomyopathy.
 
Write a written account that discusses the pathophysiological links between Hypertrophic Cardiomyopathy (HCM) and Sudden Cardiac Death (SCD) and discuss methods that the Cardiologist may use to assess John’s risk of SCD.
