BR4104 Spring Practicum
Task:
This template will help you track your practicum activities, in order to assess your progress against the goals set in your learning contract. You should fill in each section with as much detail as possible.
This template will be used to report to your supervisor during your final supervision meeting at the end of your practicum.
The completed template will then be used in a formal supervision meeting with your supervisor who will confirm or update what you have reported.
Based on your overall performance, as well as this completed template, plus your previously-submitted mid-point supervision template, your supervisor will make the recommendation that you should be awarded a “pass” or “fail” grade for your practicum.
The materials will be reviewed by UL MSc Public Health faculty, who will then discuss with your supervisor and confirm the pass or fail grade.
Note that your practicum is worth 15 ECTS credits, equating to full-time or near-full-time work over the duration of 8 weeks. You may fail your practicum if you do not provide sufficient evidence in this template of the work that you have done.
Required:
1)If your practicum started later than 22nd March, you should detail your work plan for the weeks that have not yet been completed, so that four full weeks are reported in this template.
2)Send your completed template to your supervisor during the Time.
3)Arrange a formal supervision meeting with your supervisor, lasting 30-60 minutes.
4)During your final formal supervision meeting, you will discuss the information you entered into the template.
Your supervisor will confirm or update what you have reported and, taking into account your overall performance plus your completed supervision templates, they will recommend that you should be awarded a pass or fail grade for the practicum.
5)During, or immediately after, your supervision meeting you will update your template to incorporate changes suggested by your supervisor.
As part of the assessment process, UL MSc Public Health faculty will review the completed supervision templates, and will contact each individual’s supervisor to confirm that the submitted version matches their expectations. Submitting falsified or incomplete information not approved by your supervisor may result in failure of the practicum.
